Change of name or surname

You can apply for a name and/or surname change by visiting the embassy in person (prior registration by email at kons.za@urm.lt is required).

• Conditions: The chosen name or surname must match the person's gender, not violate good morals or public order in Lithuania, and—in the case of a minor—serve the best interests of the child.
• Age eligibility: Lithuanian citizens aged 16 and older have the right to change their name or surname. For minors under 16, changes require joint agreement between both parents.

For applicants aged 16 and older:

• Application to change the name and/or surname of a person aged 16 or older. DOWNLOAD
• Valid Lithuanian passport or ID card.
• Supporting documents proving the change abroad, permanent residence abroad, or foreign citizenship. Documents (except foreign passports) must be legalized or Apostilled and translated into Lithuanian.
• Consular fee: 50 EUR (plus 20 EUR if an extract confirming the change is requested).

For minors under 16:

• Application to change the name and/or surname of a minor under 16, completed by parents. DOWNLOAD
• Valid passport or ID card of the applying parent.
• Written consent and valid ID of the other parent (free-form consent written at the embassy). If a parent does not appear, a notary-certified consent (legalized/Apostilled and translated if from abroad) or a court ruling limiting parental rights must be provided.
• Written consent from the child, if they have reached the age of 10.
• Consular fee: 50 EUR (plus 20 EUR if an extract confirming the change is requested).

Note: You may only apply for a new Lithuanian passport or ID card after your name and/or surname change has been officially processed.
